The Wanderers had come back from 3-0 down to beat Brisbane 5-4 in last week's semi-final, but another great escape seemed beyond them.
The Reds came close to adding a third early in the second half, but then they were caught in the 58th minute, when a beautiful move by Romeo Castelen and Brendon Santalab found Scott Neville, who blasted his shot past Eugene Galekovic into the far corner to give the Wanderers hope.
Adelaide thought they had restored a two-goal lead with just over 20 minutes to go, but when the ball was poked home from inside the box, it was correctly ruled out for offside.
The Wanderers poured forward in search of an equaliser, but they could not find the necessary finish.
As the game moved into injury time, the Reds made sure of their title, as Pablo Sanchez burst into the penalty area and fired a beautiful shot across Redmayne into the corner of the net for 3-1.
Both sides had lost their previous two grand finals - for the Wanderers it makes three losses in four years.
